Options for Community Living, Inc. is looking for a Residential Support Case Manager to join our Mobile Residential Support Team! An ideal candidate should meet the following requirements:
- Must be a high school graduate or equivalent.
- Bachelor’s degree in human related discipline is preferred or two years of college in a human service-related discipline and two years’ experience working with case management responsibilities in a setting with disabled or economically disadvantaged individuals and/or families preferred.
- Valid driver’s license, safe driving record, access to a vehicle and valid auto insurance is required.

Salary Range:
$42,900/year ($22.00/hour) – $54,600/year ($28.00/hour)
Salary offers will be commensurate with experience and other qualifications.
Schedule:
Monday-Friday: 37.5 hours/week with flexible hours to accommodate clients which may include evenings and/or Saturdays. Program is to provide on call as needed
- 9:00am – 5:00pm (30 minute break) or
- 8:30am – 5:00pm (1 hour break)
Location: In-person based out of our Ronkonkoma office, with field visits required

Responsibilities:
The Residential Support Case Manager provides supportive case management services to tenants with mental illness in supportive housing and provides on-site visits to scattered site locations. The Residential Support Case Manager will work with the tenant, treatment team, and the Mobile Residential Support Team (MRST) to identify issues and barriers that may prevent the individual from getting better. Through a mutually agreed goal plan the Residential Support Case Manager will assist the tenant in seeking solutions to achieve mental health and health care goals, sobriety, housing stability and to prevent where possible, emergency room usage and inpatient hospitalization.
- Provide site visits, write progress notes collaboratively with the tenant (when possible), review service plan and goals.
- Oversee medication maintenance and help develop self- medication skills.
- Provide linkages to community resources.
- Assist in securing vocational assessment services, job training and education programs.
- Provide advocacy services to gain appropriate entitlements and services.
- Intervene in situations requiring immediate attention to ensure safety of residents.
- Provide mediation, counseling, behavioral intervention and crisis management services.
- Consult and coordinate with clients’ family and providers as needed for coordinated service delivery.
